Durable agent conversation memory

Agent conversations can now store ordered native history across application restarts. The shared memory layer prevents duplicate appends, excludes failed attempts from future context, omits hidden reasoning and raw provider metadata, and includes retained history in account exports and deletion. Revision-aware caching avoids repeated message reads while the database remains the authority for ownership, deletion and the latest conversation state.
